I have recently gotten off my Buprenex shots maintnece I’ve been for 2 & 1/2 years. I stopped cold turkey and it has been a month and half and I am still in so much discomfort and pain!! I have gotten off methadone, Heroin, but this has been the worst pain and discomfort I ever felt.!! My boyfriend is a non-addict. He cannot relate my physical pain and discomfort. . First I tried over the counter, Tylenol arthritis, Ibuprofin, Tylenol sleep etc. He don’t want me even to take these. He think I will get “addicted”. Now after endless argument that I need more stronger prescribed meds. I told it will will non-narcotic and I got Ultram & he sees Narc., he flips!. It does not work, and he still tells me I am abusing it!! I am not taking Opiate meds, there is a difference!!. I need answers for him on difference from Percocet or Dorvocet. I know all to well, but to him I am my “own pharmacy”!! The pain is so bad sometimes I want to sleep & never wake up!! Thanks!!

Ultram is different from Percocet (oxycodone) in it’s MUCH lower intrinsic affinnity (its ‘sticking power) and its activity at the particular receptors in the brain. The majority of opiate drugs that we associate with highly addictive and pleasurable drugs are those that are pure Mu opiate receptor agonists….i.e. they have a HIGH affinnity and activity at the Mu opiate receptor, drugs like Heroin, Methadone, Morphine, Oxycodone are examples of these. Ultram (Tramadol Hydrochloride) on the other hand is virtualy ZERO opiate activity itself, the VERY weak activity felt when taking Tramadol is actualy from the M1 metabolite of Tramadol after first pass metabolism through the liver.
According to Goodman & Gilman’s: The Pharmacological
Basis of Therapeutics (tenth edition), the affinity of tramadol for the mu opioid receptor is 1/6000 that of morphine. Now do NOT confuse this by thinking that Tramadol is 6000 times weaker than Morphine as this is *FALSE*. In fact, on average, Morphine is 10 times as potent as Tramadol. BUT there are other factors at play here, such as Tramadol being only a PARTIALY SELECTIVE Mu agonist (Morphine being close to a pure Mu agonist) and Tramadol is also active at the Kappa receptor which is Often *NOT* assoctiated with ‘pleasure’. In a very simplistic summary Tramadol has MUCH less sticking power than morphine and much less potency than morphine. Whilst it is an opiate, it is a VERY VERY weak one that is rarely abused and used for pleasure. If you need to use something to taper off the Bupe (which I do not doubt) then this would likely be one of the much MUCH safer and less ‘abusable’ options. Just remember to taper your Ultram dose down just like methadone and Bupe.
